Agreement with ICT Communications [ABN: 88684890088 ].

 

Authority to transfer / connect services and service numbers

I, the customer, hereby apply for the supply of telecommunications services from ICT Communications (ABN 88684890088) (“We”, ICT Communications), as set out in this application and it’s attachments (“Services”), according to the terms of ICT Communication’s Full Standard Form of Agreement (SFOA), which bound by: Further, in relation to the transfer to ICT Communications of Fixed Line, Mobile, Inbound and Data Services Numbers (as set out in this application and its   attachments) from my current provider (Existing Services and Existing Service Numbers”).

I agree to the following conditions:

1. I authorise the transfer of the Existing Services and Existing Service Numbers from my current provider listed on the application and its attachments (“Current Provider”) to ICT Communications
2. I have the authority as lessee of the Existing Services and Existing Service Numbers, or as authorised agent for the lessee, to request the transfer of the Existing Service and Existing Service Numbers to ICT Communications
3. I acknowledge that transferring the Existing Service Numbers to ICT Communications may result in disconnection of the Existing Services (including services related to the Existing Service Numbers) and may also result in finalisation of my account with my Current
4. The Existing Services and Existing Service Numbers will be transferred with their current I acknowledge that by transferring my Existing Services and Existing Service Numbers I may lose services, functions and facilities provided by my Current Provider and that new services will be provided by ICT Communications.
5. I acknowledge that any incentives and benefits I may receive from my Current Provider in relation to my existing Services will not be available after the transfer to ICT Communications
6. The Existing Services and Existing Service Numbers will remain active with my Current Provider and my Current Provider will continue to provide me with the Existing Services until the transfer to ICT Communications has been
7. I will be responsible to my Current Provider in relation to any changes incurred prior to my Existing Services transferring to ICT Communications
8. I acknowledge that I may have rights and obligations to my Current Provider after the transfer of the Existing Services and Existing Service Numbers to ICT Communications has been completed and that is my responsibility to check the terms and conditions of my existing
9. Existing Service Numbers that are not on Telstra Network will incur a one off porting fee of up to $165.00.
10. New line connections will incur a one off connection fee of up to $367.00 as stated in Critical Information
11. I will contact my Current Provider in relation to any faults until such time as the transfer of my Existing Services is
12. I understand that my Current Provider allows ICT Communications to request my current network provider to access and use the account numbers relating to my Existing Service Numbers for the purposes of transferring my Existing Services to ICT Communications
13. Who, and any other carrier or supplier of telecommunications services, may exchange call charges, account and other You appoint us as your agent for this purpose and to action any necessary transfer process and to complete any documentation for these purposes.

16. For fixed line service -This is a 12 month contract whereby early termination will incur a $250 Exc GST early termination charge.
17. For mobile service -This is a 24 month contract whereby early termination is remaining months of the contract x monthly fee.
18. We reserve the right to churn back customers who have transferred away unless written/email notice is provided to ICT Communications one month in advance regardless of whether the client is on a contract with us or not. Any usage charges incurred during this one month period will be payable to ICT Communications.
